Music, from its earliest presence in humanity, has served as a language for communicating emotions, ideas, and deeper human desires that words cannot fully convey (Psaltopoulou, 2005). As a language, music is composed of laws and specific structures to facilitate the conveyance of meaning. In essence, it is a living organism, taking on an active role as a subject rather than as an object. For this reason, Small (1998) notably calls music "musicking," suggesting an active process of creating music. Music simultaneously touches humans on emotional, spiritual, physical and intellectual levels, since sound vibrations, whether heard or not, move and resonate with the body, creating a kind of rhythmic embrace. At the same time, it contains an artistic element, often spontaneous, perhaps unexpected and rich, transcending everyday experience to evoke surprise, astonishment, and a sense of transcendence. Music itself—and even more so music therapy—can be seen as a three-part family, where structure and laws represent a paternal role, the rhythmic embrace evokes a maternal sense, and the art takes the place of an imaginative and creative child (Psaltopoulou, 2012). This chapter begins with an introductory outline, illustrating a form of connection and communication among the four elements of nature, weather conditions, four states of human psychopathology, and music, highlighting music's remarkable capacity to make humans inherently musical beings. Following this, the chapter describes and analyzes communication within interactive music therapy. Although primarily humanistic, this is presented through a Lacanian-psychoanalytic perspective on the structure of the psyche, linking it to the clinical use of music and a new form of expression FA-fonie (FA-voice) that emerges in individuals with a psychotic structure of the psyche during music therapy sessions, as well as in Community Music Therapy, guiding individuals toward more functional communication with themselves and others. The therapeutic approach in music therapy is primarily interactive-creative. The topic is illustrated with a case presentation. The chapter also includes a brief presentation of the four types of discourse, as identified and named by Lacan, outlining four types of communication also observed in music therapy (Psaltopoulou, 2014a, b, c, d, e, f). This topic is explored through three brief case studies, along with a fourth presentation of primary material containing the words of an individual who sought music therapy as a therapeutic intervention for personal issues.
