Sports injuries of the upper extremity are a common entity among athletes with different clinical features and distribution depending on the sport and population. Various classifications are used for sports injuries, with the main one classifying them into acute and chronic. Symptom duration of 6 weeks to 6 months has been proposed to characterize an injury as chronic, with great heterogeneity and subjectivity existing in the precise definition of symptom duration. It is often preferable to use the term "overuse syndrome" which describes conditions that are not caused by acute injury, but by repeated minor injuries and continuous strain. Overuse injuries mainly involve tendon conditions, such as the rotator cuff or the elbow extensors. Shoulder injuries are particularly common in throwers and in athletes who perform frequent overhead shoulder movements. Major injuries of the shoulder include subacromial impingement, rupture of the rotator cuff, and shoulder instability with labral tears. The elbow is most commonly injured in throwing sports such as baseball and tennis. The majority of these injuries are chronic, resulting from overuse, with the main manifestation being external epicondylitis. Sports injuries of the wrist and hand constitute a special challenge for the orthopedic surgeon and require a personalized approach according to the needs, sport, and age of the patient. They most commonly involve injuries to bony structures (fracture of the distal metacarpals, metacarpals, and fingers) or ligamentous injuries. The diagnosis of all injuries is set clinically and is confirmed radiologically. When the diagnosis is made, the therapeutic approach begins taking into account the nature of the injury as well as the professional level of the athlete. When conservative treatment is chosen, the measures applied include anti-inflammatory drugs, physical therapies, and the utilization of special splints. Rehabilitation-physical therapy plays an important role in the upper extremity injuries. The goals of rehabilitation include reduction or elimination of pain, improvement of mobility by regaining full range of motion, control and reduction of the swelling, full strength and muscle synergy recovery.
