Abstract:
Tendon injuries and tissue defeccts in the hand are special injuries and appropriate management is a challenge to the Orthopaedic Surgeon. In the last decades, treatment of these injuries has been improved significantly both in repair techniques and rehabilitation protocoles. To achive a satisfactory outcome, the surgeon must have a deep knowledge of the biologic principles of tendon healing, the normal anatomy and histology, the operative repair techniques, the special tendon characteristics based on tendon zones, and the rehabilitation protocoles. Regardless of the type or level of the injury and regardless if the injury includes flexor or extensor tendons, the ultimate aim of the surgeon and physiotherapist is protection of the repair, prevention of adhesions, maintenance of joint motion and the best excursion of the tendon. Endorsement of early mobilization during rehabilitation has reduced complication rate and has improved tendon healing. Appropriate selection of the immobilization splint and rehabilitation protocole are also equally important with performance of the repair tehnique. Regarding skin defects, alternative coverage methods are presented, from the simplest to the complex tissue repair techniques with flaps.
