Abstract:
Brachial plexus injuries affect young adults (usually due to traffic accidents) but also newborns (usually due to plexus injuries during childbirth: neonatal or obstetric brachial plexus paralysis). In infants, the majority of lesions have a good prognosis with a high rate of spontaneous recovery, and the recovery of biceps function within the first three months of life is a positive prognostic sign. However, when root avulsion or injury to multiple roots is present, the prognosis is poor and the patient presents with significant disability. Treatment of brachial plexus injuries involves microsurgical repair of the brachial plexus shortly after the injury. The type of surgical repair varies, depending on the type of paralysis (superior or total type) and the type of nerve damage (transection or avulsion), and primarily includes neurolysis, bridging using nerve grafts, and nerve tranfers. Secondary rehabilitation aims to improve functional outcome and includes skeletal interventions, muscle transfers and releases, and tendon transfers. Regarding peripheral nerve injuries the etiology and classification of injuries are analyzed as well as the assessment of the patient with peripheral nerve injury, prognostic factors and methods of nerve rehabilitation (end-to-end and end-to-side nerve suturing, bridging deficits with nerve grafts and conduits, nerve transfers and secondary rehabilitation procedures).
