ΟΞΕΙΑ ΛΕΜΦΟΒΛΑΣΤΙΚΗ ΛΕΥΧΑΙΜΙΑ

Abstract

Acute lymphoblastic leukemia is the unjustified, unnecessary and uncontrollable proliferation of the lymphoid series with a complete block of differentiation at the stage of blasts. In this instance, blastic cells infiltrate the marrow and come out in the circulation invading and disturbing the organs of the body and ultimately causing death of the patient. Acute lymphoblastic leukemia (ALL) is common in children (with a relatively good prognosis) and in old age (with an unfavorable prognosis). There are many types of ALL with varying severity and prognosis. Their classification is based on several morphologic, chromosomal, immunophenotypic and molecular criteria, which are useful prognostic factors and have great value for the determination of the appropriate therapeutic approach. Therapy of ALL is complex and long lasting. However, it may end up with complete cure in a high percentage of infantile/ childhood ALLs, in contrast to the adult patients whose therapy is also difficult but disappointing. The therapeutic approach comprises complex combinations of steroids with various cytostatics and is completed, when the correct indications apply, with allogeneic stem cell transplantation.

Αναίτια, άσκοπη και ανεξέλεγκτη υπερπλασία της λεμφικής σειράς με πλήρη αναστολή της διαφοροποίησης. Τα βλαστικά κύτταρα διηθούν τον μυελό και ξεχύνονται στο αίμα, για να διηθήσουν στη συνέχεια άλλα ευγενή όργανα και να επιφέρουν τον θάνατο. Συνήθης στα παιδιά (με καλή πρόγνωση) και στις μεγάλες ηλικίες (με κακή πρόγνωση). Πολύπλοκη κατάταξη που βασίζεται σε μορφολογικά, καρυοτυπικά, ανονοσοφαινοτυπικά και μοριακά κριτήρια, τα οποία έχουν προγνωστική αξία, και μπορούν να καθοδηγήσουν την επιλογή της καταλληλότερης αγωγής. Η τελευταία είναι επίπονη και χρονοβόρα, μπορεί όμως να επιφέρει ίαση σε πολύ υψηλά ποσοστά των ΟΛΛ της παιδικής ηλικίας, όχι όμως και στις ΟΛΛ των ενηλίκων, όπου η πιθανότητα ίασης είναι πολύ μικρή. Η αγωγή περιλαμβάνει πολύπλοκους συνδυασμούς κορτικοειδών με άλλα κυτταροστατικά φάρμακα και συμπληρώνεται, όταν υπάρχει η κατάλληλη ένδειξη, με μεταμόσχευση αλλογενών στελεχιαίων αιμοποιητικών κυττάρων.
