Τhis chapter discusses the use of mechanical ventilation on a long-term basis for those patients, who cannot be successfully weaned from the ventilator. This type of mechanical ventilation involves two general categories of patients. Primarily, those patients recovering from an acute illness that required mechanical respiratory support. These patients should be transferred to special units, even within the same hospital (long-term hospitalization units or intensive care units), or transferred to their home, once the acute phase of their main disease has passed and is resolved. Secondly, patients suffering from a chronic and progressive disease that requires mechanical support of breathing, such as patients with a type of obstructive lung disease, e.g., Chronic Obstructive Pulmonary Disease (COPD), or with restrictive type diseases (e.g., kyphoscoliosis), which are essentially incurable, and their course is usually progressive.
