Mechanical ventilation causes cardiopulmonary interactions that are relatively complex. The applied tidal volume, and consequently airway pressures, affect both right and left ventricular preload and afterload, resulting in reduced thoracic venous return and better left ventricular ejection. At the same time, increased pressures in the alveoli result in increased pulmonary resistance (due to external capillary compression) and have a negative impact on the function of the right ventricle, especially if it is already dysfunctional. The exact opposite occurs when intrathoracic pressure decreases. Correct assessment of cardiopulmonary interaction is imperative in critically ill patients, since mechanical ventilation can impose a dramatic hemodynamic burden that even affects ICU mortality. The clinician should titrate airway pressure to optimize arterial oxygenation and cardiac output, minimizing pulmonary vascular resistance, regularly monitoring cardiorespiratory parameters.
